  • To see a blank copy of the Clerk's Annual Questionnaire (in Adobe Acrobat format) before you submit your answers click here: View Survey. For best results, right-click the "View Survey" link (or click and hold for Macintosh), select "save target as" and save the document to your desktop for viewing and printing.


  • When you have completed the survey and submitted your data, you can return here and log in again to view or change your answers, if necessary.


  • To fill out survey you will need your congregation's PIN. Please take a moment to locate your congregation's PIN before you proceed. Here are three sources for your PIN:

    1. Your congregation's PIN is printed at the top of the paper copy of the Clerk's Annual Questionnaire that you received from your presbytery.

    2. Your pastor, church secretary, or presbytery office can also help you find your PIN.

    3. You can call PresbyTel, the denomination's information service, toll-free at (800) 872-3283 to get your PIN (Monday through Friday 9 a.m. to 6 p.m. Eastern Time).
