BOP extends deadline to take Mayo Clinic Health Risk Assessment

$25 gift card offered for completing online survey by Nov. 30

LOUISVILLE — The Presbyterian Church (U.S.A.) Board of Pensions (BOP) has extended the deadline on a special promotion that encourages active BOP plan members and their spouses to take an online Health Risk Assessment administered by the Mayo Clinic.

Those who take the assessment can discover health risks they may not realize they have and qualify for a $25 gift card reward from the BOP.

“Due to the positive response we have received to the Mayo Clinic Health Risk Assessment, the Board of Pensions and the Mayo Clinic are extending the deadline for taking the assessment to Friday, November 30, 2007,” the BOP said in a statement issued Nov. 19.

Those wishing to take the online assessment and receive the gift card must do so by 11:59 p.m., Central Time, on Nov. 30.

Two announcements about the Health Risk Assessment have been mailed to all eligible members and their spouses.
