Thank you for bringing this to our attention. There is a coding error in the form that is preventing those two fields from accepting paragraph text. We will correct the coding and post an update soon.

When saving this file as a pdf, the form box for question 1 is fine but 2 and 3 the box is too small to write the required statement. The creator of this form needs to make the boxes bigger. I can't write a paragraph in a sentence space.
