Form 3 - Pre-consultation report on development areas
This report, completed by the inquirer/candidate before each consultation, evaluates progress in accomplishing previously agreed-upon goals and objectives. The form requests a listing of completed courses, and includes specific questions to be addressed in each of the five growth areas.

Why are the boxes for the responses only allowing to write 5 lines?

As is explained on the "index" page to the forms, these files use some of the most recent features of Adobe Acrobat. They work best with the actual Adobe Reader (not Apple Preview or other PDF creators) require the use of version 9 or higher. If either the original editor of the form or the recipient of the completed form use other PDF readers, they often have difficulties like those described.

I am having lots of issues with the filled in forms sent to me by our Candidates and their info either completely disappearing or no info visible until I click on a field. Is anyone else have difficulties with these forms? Do you have any fixes?
